Two models. One chart. Every note generated by ChartPilot is independently audited before you can sign. The auditor checks the prose against the structured facts extracted from the visit, against deterministic rules (E/M level, medical necessity, denial patterns), and produces a QA report attached to the encounter. AI Narrative Mode lets you choose how the note reads — concise, detailed, or in your own dictated voice via Voice DNA, which adapts to each provider's phrasing over time.

A contraindications engine screens patient allergies, medications, history, eGFR, and pregnancy with absolute and relative gating and required override notes. Orders export as paginated PDF requisitions.

ChartPilot is built around the assumption that an AI-drafted note is a draft until something independent has checked it. Before a provider can sign, the system runs three layers of review:
- Layer 1 — Fact grounding.
The note is regenerated against a structured facts JSON extracted from the visit's source inputs. Anything in the prose that isn't in the facts is flagged.
- Layer 2 — Deterministic rules.
A rules engine applies CIGNA CPG-278, medical-necessity language requirements, E/M level checks, and a denial-pattern validator built from real payer rejections.
- Layer 3 — Independent auditor.
A second model (Claude) audits the first model's (GPT) output against both the facts and the rules and produces a QA report artifact attached to the encounter.
If anything in those three layers fails, signing is gated until it's resolved or explicitly overridden — and the override is captured in the audit log. This is the same posture FDA Clinical Decision Support guidance asks of any AI that influences clinical documentation.
